One of the most challenging aspects of programming a Personal Health Record is developing a longitudinal record to use as a baseline. Rather than specify what functionality should be included the PHR, this implementation guide endeavors to define patient persona benchmarks that PHRs should be able to either consume or generate.
It is our hope that your applications should be able to load any of the following PHR files and not error out or fail. That is not to say that your application ought to use all of the data in the following records; rather, it simply needs to be able to parse the record-keeping of a file format that can contain large DICOM files, crypotographically signed provenance records, patient data receipts, and other recordkeeping structures that we see in the PHR ecosystem.

For additional research on patient narratives and the circumstances that shape many of the most challenging clinical cases, particularly with regard to social determinates of health, the Soft White Underbelly videos may be of interest. It often helps to hear accounts from a first-person perspective of the varied circumstances that lead to homelessness, incarceration, chronic illness, mental health episodes, disability, etc.
