StructureDefinition-endpoint-reference
An extension to support the addition of a reference to an endpoint for the practitioner and careTeam resources.
An individual may have an electronic endpoint independent of their role (e.g. a personal Direct address).
A care team may have an electronic endpoint independent of the members of the care team, location where the care team operates, services it provides, etc.
