Understanding FHIR

This implementation guide is based on the HL7 FHIR standard. It uses terminology, notations and design principles that are specific to FHIR. Before reading this implementation guide, it is important to be familiar with some of the basic principles of FHIR as well as general guidance on how to read FHIR specifications. Readers who are unfamiliar with FHIR are encouraged to read (or at least skim) the following prior to reading the rest of this implementation guide.

Feel free to explore other aspects of the FHIR specification that you feel may be relevant or of interest.

Resources

This implementation guide defines profiles on several resources. It also leverages profiles from underlying guides. Where implementers adhere to one of the profiles defined in this guide, they will also need to adhere to the base expectations defined in the profiled resource. The table below lists all resources used as part of the current IG and indicates where they are used, as well as which profiles apply. Implementers are strongly encouraged to read through the introductions and implementation notes associated with each resource they will implement, though not all resource guidance will necessarily apply in the context of implementing this guide.

Practitioner An individual involved in the care delivery process, independent of what organization they work for or what 'hat' they are wearing. PractitionerRole Someone involved in the care delivery process with the ability to identify the specific individual, the organization they are working for, and/or the 'hat' they are wearing. Questionnaire Allows sharing a specific form to be filled out by a patient or caregiver as part of the care coordination process. Used as part of the Patient Task process described in the SDOH Patient Coordination Workflow. SDC Base Questionnaire SDC

RelatedPerson Allows capturing the participation of caregivers such as family members or neighbors (rather than only the patient) in the process - as authors, performers, subjects, etc. For more information on the involvement of caregivers, see the Design Considerations page. Service Request Can be used to represent either an exercise prescription - an order for physical activity directed at the patient, or a referral - a request for another provider or organization to assist with the patient's physical activity in some way. PA Physical Activity
Task Used to monitor the process of asking someone to do something, as well as tracking the status of the resulting execution and providing any results of the action back to the requester. Used when seeking actions by a patient or caregiver as part of the SDOH Patient Coordination Workflow, as well as when asking a Service Provider to fulfill a referral.
