HSDSHealthcareService

The HSDSHealthcareService resource describes the social and human services offered by Community-Based Organizations (CBO) at a given location. This resource may be used to encompass a variety of human and social care service interventions that assist patients and clients with unmet social needs. Examples include food, housing/shelter, income & employment, public transportation, public education, legal services, disability and aging and mental and physical health.

HSDSLocation

The HSDSLocation resource describes the physical place where community-based services are provided, practitioners are employed, organizations are based, etc. Locations can range in scope from a room in a building to a geographic region/area.

HSDSOrganization

The HSDSOrganization resource is a formal or informal grouping of people or organizations set up to assist people in coping with issues related to various social issues, including but not limited to: adequate housing, substance abuse, domestic conflict, mental health and/or personal/familial problems. Guidance: When the contact is a department name, rather than a human (e.g., patient help line), include a blank family and given name, and provide the department name in contact.name.text.

These define sets of codes used by systems conforming to this implementation guide.

Human Services Category Value Set

This example value set is intended to represent the high level categories corresponding to human and social services domains. The value set includes values descending from Level 2 of the 211 Human Services Indexing System, except codes included in Level 1 (Y - Target Populations).

Human Services Type Value Set

This example value set represents the more refined classification of human services by which human services directories can be queried. This value set represents codes found in Levels 3-6 of the 211 Human Services Indexing System, except codes descending from Level 1 (Y - Target Populations).

211 Human Services Indexing System

The 211 Human Services Indexing System, formerly known as 211 LA Taxonomy of Human Services, is the North American standard for indexing and accessing human services resource databases. The Taxonomy is a hierarchical system that contains more than 9,000 fully-defined terms that cover the complete range of human services.
