Dental Bundle |
This profile on FHIR’s Document Bundle paradigm is applicable to the exchange of Referral Notes and Consult Notes and requires at a minimum the first entry be Dental Referral Note or Dental Consult Note Composition profiles, respectively. All other referenced resources are optional and encouraged. |
Dental Referral Note |
This Profile defines the Dental Referral Note profile. It contains the textual information regarding a referral to a general dentist or dental specialist as well as references to supplemental dental profiles for the referral |
Dental Service Request |
This profile contains information regarding the dental referral request including a required identifier and a reference to the reason for the referral, if possible. It also includes basic information such as the patient, referring provider, and encounter information that resulted in the referral. |
Dental Consult Note |
This profile defines the Dental Consult Note. It contains the textual information regarding what was observed and performed by a general dentist or dental specialist in response to a dental referral as well as references to supplemental dental profiles for that referral. This Dental Consult Note is linked to its Dental Referral Note via the In-Fulfillment-Of extension reference to ServiceRequest. |
Dental Condition |
This is the Dental Condition profile. It is intended to be used to distinguish dental-related problems from purely medically-related problem (US Core Condition). |
Dental Finding |
This profile is used to convey oral findings including from odontograms, periodontal assessments, tooth-specific observations, intraoral and extraoral soft tissue exams, TMJ, TMD, and occlusion assessments. Findings in this profile often support associated diagnoses (FHIR Conditions). Dental measurements are represented here as observations. Measurements are distinct from a dental problem which would be represented in the Dental Condition profile. |
Dental Communication |
This profile is used to convey verbal and written communication and education between providers, patients, and other members of the care team. |
