The practioner role extension is used whereever there is a reference to the FHIR Practioner resource. It's purpose is to overcome a shortcomming the FHIR STU3 that was addressed in FHIR R4. In FHIR STU3 it is not possbile to reference both the practioner and the practioner role in a single reference. This has been addressed in FHIR R4 by allowing practioner role to be referenced whereever there a reference to practioner is allowed. Pracitioner role then includes a reference to the practioner.
This is a critical requirements in BSeR to know not only who the practioner is but wheather the practioner is the referral recipient or the referral initiator.
