      .descriptionThe US Core Observation Social History Profile meets the U.S. Core Data for Interoperability (USCDI) v2 ‘SDOH Assessments’ requirements. To promote interoperability and adoption through common implementation, this profile sets minimum expectations for the Observation resource to record, search, and fetch retrieve observations that represent simple observations made by an individual about a patient's social history status such as education, food insecurity, etc. It identifies which core elements, extensions, vocabularies and value sets **SHALL** be present in the resource when using this profile. It provides the floor for standards development for specific uses cases.This Observation profile represents potential patient exposure and contact information. The type of exposure/contact is contained in Observation.code (environmental, activity, event, location, person, animal, etc.). If the exposure/contact is an entity (person, animal, location) then it is represented in Observation.focus which is a reference to a Patient, RelatedPerson, Location, Group, etc.. (An animal would be represented by a RelatedPerson that contains the Extension Practitioner Animal Species. Free text information about the exposure/contact can be recorded in Observation.valueString and the date or period of exposure is held in Observation.effective[x]. If an address where the exposure/contact took place is known then it can be contained in the address-extension.
