The PCO IG defines profiles for interoperable goals, leveraging Goal Attainment Scaling (GAS) or Patient-Reported Outcome Measures (PROMs) to track progress. GAS involves collaboratively setting goals with numerical scales to assess achievement levels, offering a nuanced and individualized measure of progress. PROMs, selected based on goal focus or barriers, provide structured patient-reported data to quantify outcomes where traditional measures fall short. |
