The date (and optionally time) when the {{title}} was published. The date must change when the business version changes and it must change if the status code changes. In addition, it should change when the substantive content of the {{title}} changes.

Note that this is not the same as the resource last-modified-date, since the resource may be a secondary representation of the {{title}}. Additional specific dates may be added as extensions or be found by consulting Provenances associated with past versions of the resource.

Unique id for the element within a resource (for internal references). This may be any string value that does not contain spaces.

Source of the definition for the extension code - a logical name or a URL.

The definition may point directly to a computable or human-readable definition of the extensibility codes, or it may be a logical URI as declared in some other specification. The definition SHALL be a URI for the Structure Definition defining the extension.

Value of extension - must be one of a constrained set of the data types (see Extensibility for a list).
