Operation Outcome records are transient items that are returned in response to specific interactions or operations. Contraindications are items that can be stored in the paitient's records as reasons that things were not done (or done). This extensions allows a transient operation outcome to refer to a persistent entry in the patient record as the basis for the outcome.
