      <p>The condition, state, or problem that the patient is in or has prior to a therapy or procedure.
         This captures temporal (temporary circumstances) that have bearing on the data that it
         qualifies but will not necessarily modify its meaning. Things like 'after activity', 'at
         rest', or 'post-op'. </p>
